Pool Exercises for Arthritis: A Senior's Guide to Pain-Free Movement

For seniors with arthritis, exercise can feel like a cruel paradox: movement is the best treatment for joint stiffness and pain, but movement causes joint stiffness and pain. Water changes this equation entirely. In a warm therapeutic pool (33-36°C), buoyancy reduces the load on your joints by 50-90%, hydrostatic pressure reduces swelling, and the warmth relaxes muscles and eases stiffness before you begin moving.

1. Water walking

Target: Full body, cardiovascular endurance

How to do it

Walk forward across the pool in waist-to-chest-deep water. Take normal-length steps, swinging your arms naturally. Walk for 2-5 minutes, then walk backward for 1-2 minutes. The water resistance strengthens muscles while buoyancy protects joints.

Why this works for arthritis

Walking in water allows a natural gait pattern that many arthritis patients can't achieve on land due to pain. The warm water relaxes stiff muscles before you begin moving.

2. Leg swings (hip mobility)

Target: Hip joints, hip flexors, glutes

How to do it

Stand in chest-deep water, holding the pool edge for stability. Swing your right leg forward and backward in a pendulum motion, keeping it straight. Do 10-15 swings, then switch legs. Next, swing each leg out to the side and back. 10-15 reps each leg.

Why this works for arthritis

Water buoyancy supports your leg weight, allowing a greater range of motion than you'd achieve on land. This is particularly beneficial for hip osteoarthritis.

3. Underwater arm circles

Target: Shoulders, rotator cuff

How to do it

Stand in shoulder-deep water with arms extended to the sides. Make small circles with your arms (about 6 inches diameter), gradually increasing to larger circles. Do 15-20 circles forward, then 15-20 backward. The water provides constant, gentle resistance.

Shoulder arthritis often limits overhead movement. In water, the buoyancy assists upward movement while resistance on the downward phase builds strength without joint compression.

4. Knee lifts with resistance

Target: Hip flexors, quadriceps, core

How to do it

Stand in waist-deep water, holding the pool edge. Lift your right knee toward your chest, then extend your leg forward (like a slow kick). Lower it slowly. The water resistance on the lowering phase is the strengthening component. 10 reps each leg.

The slow, controlled movement in water builds quad strength without the joint impact of land-based knee exercises. This is critical for knee osteoarthritis management.

5. Side stepping

Target: Hip abductors, inner thighs, lateral balance

How to do it

Stand in chest-deep water facing the pool wall. Step sideways along the pool edge, leading with your right foot, then bringing your left foot to meet it. Take 10-15 steps in one direction, then reverse. Keep your core engaged and move deliberately.

Strengthens the hip muscles that stabilize your pelvis during walking, critical for arthritis patients who develop a compensatory gait due to pain.

6. Water squats

Target: Quadriceps, glutes, hamstrings

How to do it

Stand in chest-deep water with feet shoulder-width apart, back against the pool wall. Slowly bend your knees, lowering your body into a squat position (knees at 45-90°). Hold for 2-3 seconds, then push through your heels to stand. 8-12 reps.

On land, squats can be agonizing for arthritic knees and hips. In water, buoyancy reduces your effective body weight by up to 90%, making the exercise comfortable while still building the strength needed for getting up from chairs.

7. Wrist and finger exercises

Target: Hand joints, grip strength

How to do it

In waist-deep water, submerge your hands. Open and close your fists 15-20 times. Spread your fingers wide, then bring them together (15 reps). Make circles with your wrists in both directions (10 each way). Finish by squeezing a soft aquatic ball underwater for 10 reps each hand.
